For your office visit, please bring all available medical records including doctor office records, hospital records, x-rays, CT (computed tomography) scans, MRI (magnetic resonance imaging) scans, and ultrasound scans.

As the patient, you may have to sign a HIPAA release form (Health Insurance Portability and Accountability Act), which protects your privacy. The physician cannot obtain your medical information without your written permission.
